A New Chapter in Community Health Care: Hudson Headwaters Expands Access in Malone

For over four decades, Hudson Headwaters Health Network has been a trusted name in health care across Upstate New York. Now, the nonprofit organization is deepening its commitment to the North Country with the opening of a new primary care practice in Malone.


Family Health at Malone officially opened its doors in December 2023. Conveniently

located on the Alice Hyde campus at 16 3rd Street, the practice is already making an impact by welcoming patients of all ages, regardless of insurance status or income level.


“Access to primary care is a foundational part of a healthy community,” said Susan Foster, FNP, one of the practice’s providers and a Malone resident. She is joined by fellow provider April Worlock, FNP, who also lives in Malone. Both are passionate about serving their neighbors and addressing the critical shortage of local primary care providers. Additional providers are actively being recruited.


The new facility offers a full range of services, including routine physicals, chronic disease management, family planning, care coordination, preventive screenings and integrated behavioral health care. Same-day appointments are available, and financial assistance programs help ensure that no one is turned away due to cost. New patients can call 518-824-2562 or schedule online.


Building for the Future

Family Health at Malone is just the beginning. Hudson Headwaters has broken ground on a much larger, state-of-the-art health center that will further expand access in the region. Scheduled for completion in early 2026, the new Malone Family Health facility will feature 15 exam rooms in a 13,000-square-foot building designed to meet the community’s evolving health needs. Malone Family Health will be located at 24 4th Street in Malone.


“We’re not just opening a building—we’re investing in the long-term wellness of Franklin County,” said Hudson Headwaters CEO, Tucker Slingerland, M.D. “Alongside our many partners in Malone, we’re focused on ensuring residents have consistent, local access to high-quality health care.”


Expanding Access with the 340B Federal Drug Pricing Program

This new facility and the expansion of primary care access in Malone would not be possible without the 340B Federal Drug Pricing Program. Hudson Headwaters Health Network depends on this Program to ensure our communities have affordable access to health care. 340B funding is required to be reinvested directly into patient care, supporting services, staffing, and infrastructure that wouldn’t otherwise be feasible.


“Malone Family Health is a perfect example of 340B dollars in action,” explained

Dr. Slingerland. “This essential funding means the Network can be there when communities like Malone need us.

Rooted in Community

To help ensure Hudson Headwaters meets community needs, the Network is currently seeking members for its Malone Patient Advisory Committee. A strong candidate for this role is an enrolled Hudson Headwaters patient or caretaker of a patient, who receives care in Malone. Committee members help the Network understand what’s working well and opportunities to improve.
